Safety/Discipline Report Form
 

Dear UFT Member,


Our members and the students we educate have the right to a safe and secure school/site environment. Unfortunately, that right is not self-enforcing. To protect it, the first step is to report every safety problem in your school or work site. That’s why the union is introducing this online reporting system as a way for you to easily make a report.


Remember that you should also report a safety problem to your chapter leader and to your supervisor via a DOE school occurrence report. Make sure you keep copies of all forms.


If you believe the situation you are reporting constitutes an emergency, please call the union’s safety hotline at (212)701-9407 for immediate assistance.
