Quality of Life Preferences Survey

You are invited to participate in our survey on Quality of Life preferences.

In this survey, those who have expressed an interest in Lofton Consulting strategies will be asked to complete a short survey that asks questions about Quality of Life preferences.

It will take approximately five (5) minutes, or less, to complete the questionnaire.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point.
