You are invited to participate in our Child Center Questionnaire survey! In this survey, you will be asked questions about your center's television and video watching habits. It will take approximately 15 minutes to complete the questionnaire.



Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. Please answer each question to the fullest extent possible (for example, put N/A if the question does not pertain to you). We are very excited about having you participate so we may gain a wider understanding of infant and toddler media habits.



Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
