To Group the survey in to sections and to enhance the look of the survey you can add Section/Page Headers to the survey.
To add Section/Page headers in the Survey: Go to the Edit Survey page: Click on the Add New Survey link for the question, after which you want the Section/Page header.

From the Question Type drop-down menu select the Section/Page Header option under Presentation Text (Display Only) and click on the Add Question button.

Follow the directions in the popup wizard to add a Section/Page Header to the Survey. The Section/Page header will look like this:
